Vista’s healthplans for individuals offer coverage for preventive care, prescription drugs, hospitalization, dental & vision, immunizations, mammograms, maternity, among others. For children under six months of age or older, Vista offers child-only health plans.
To apply for a Vista benefit plan download an application from the website and mail it with your signature. You can also receive help for same from agents or known authorized offices. The forms can be posted to “Vista Individual, P.O. Box 191999, Plantation, FL 33318”. Applicants without a Social Security Number or of age 50-64, need to send in their medical records along. Each applicant is also required to select a primary care physician (PCP) from the provider directory. Note that a personal check must be attached for one month’s premium made payable to Vista.
