You can print and fill out the Name Change Record Check form and submit via mail or in person to Toronto Police headquarters. Online applications will receive a response in 7-10 business days. A confirmation number will be displayed if the application has been accepted. The Criminal Record Check document or Clearance Letter will indicate if a person has any criminal convictions. Involved in a collision? You can visit a Collision Reporting Center (CRF) to report the incident. There are currently two Collision Reporting Centres – East Collision Reporting Centre and North Collision Reporting Centre.
