About Blue Cross Community Family Health Plan

Provided by Blue Cross and Blue Shield of Illinois, a Division of Health Care Service Corporation, the program includes coverage for preventive care services, emergency and urgent care coverage and diagnostic tests. Members get access to primary care physicians as well as pediatricians and specialists. The plan is currently available only in six Illinois counties: Cook, DuPage, Kane, Kankakee, Lake and Will.

Access to a doctor or healthcare provider is absolutely free as part of the plan. Services by non-licensed provider without a prescription is not covered. Those needing behavioral health services can get mental health assessment or therapy/counseling. Additionally, the Blue Cross Community Family Health Plan includes coverage for selected prescription and over-the-counter (OTC) drugs and selected medical supplies.

Note that Blue Cross Community Family Health Plan members also qualify for Long Term Services and Support (LTSS). To be eligible one must be a resident of the state of Illinois, have a Determination of Need (DON) score of 29 points, less than $17,500 in assets and cooperate with the Medicaid application process. They must also be in the ages of 19 and 64.

Those who are enrolled in the Illinois Medicaid program and part of the FamilyCare or Moms & Babies program can join Blue Cross Community Family Health Plan. Those who qualify for Medicare in addition to Medicaid cannot enroll in the program.
